  2. Fill out the Camp Contract of Agreement or print and mail in.
  3. TO FINALIZE REGISTRATION:  Please note Registration is not complete until
    form and payment is received.  Pay Online if you wish or print the forms and
    mail with payment to: Nutrition Innovators, 509 Rue de Belier, Lafayette LA

  4. Print a copy of the Physician's Form and have your child's doctor complete and
    sign the form. Mail in to Nutrition Innovators before camp. ALL paperwork must
    be completed before camp.
